Het onderzoeksteam demonstreerde een AI die mannen met beperkt sperma kan helpen biologische kinderen te verwekken.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Momenteel moeten deze pati\u00ebnten een procedure ondergaan waarbij een deel van hun testikels wordt verwijderd, waaruit embryologen handmatig sperma halen. Dit sperma wordt vervolgens gebruikt om de eicellen van de partner te bevruchten via een Intracytoplasmatische Sperma Injectie (ICSI)-behandeling. <\/span><\/p>\n<p><span style=\"font-weight: 400\">Het huidige proces is omslachtig en tijdrovend, duurt vaak tot 6 uur en kan leiden tot vermoeidheid die de nauwkeurigheid van de sperma-identificatie be\u00efnvloedt.<\/span><\/p>\n<p><span style=\"font-weight: 400\"> Het proces heeft een hoog mislukkingspercentage en moet vaak worden herhaald.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Het onderzoek, geleid door professor Majid Warkiani van de UTS School of Biomedical Engineering, presenteert een AI-hulpmiddel dat bekend staat als SpermSearch en dat dit belastende proces versnelt.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Hoofdauteur Dale Goss, een promovendus, verklaarde: \"Dit hulpmiddel heeft de mogelijkheid om pati\u00ebnten die heel weinig kans hebben om hun eigen biologische kinderen te verwekken, een grotere kans te geven.\" <\/span><\/p>\n<p><span style=\"font-weight: 400\">Hij voegde eraan toe: \"Het algoritme verbetert verouderde benaderingen die al tientallen jaren niet meer zijn bijgewerkt. Het zal zorgen voor een snelle identificatie van sperma in monsters, wat niet alleen de kans vergroot dat een koppel hun eigen biologische kinderen verwekt, maar ook de stress op sperma vermindert en de effici\u00ebntie in het laboratorium verhoogt.\"<\/span><\/p>\n<h2>Hoe het werkt<\/h2>\n<p><span style=\"font-weight: 400\">De onderzoekers benadrukten dat ongeveer 1% van alle mannen, ongeveer 5% van de koppels die een vruchtbaarheidsbehandeling zoeken, lijden aan niet-obstructieve azo\u00f6spermie (NOA) - een ernstige vorm van onvruchtbaarheid die wordt gekenmerkt door de afwezigheid van sperma in sperma.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Embryologen identificeren momenteel sperma voor ICSI door weefselmonsters nauwgezet te versnipperen en te scheiden met een tang of fijne naalden. Eventueel aanwezig sperma wordt dan losgelaten in een speciaal geprepareerde vloeistof, in een petrischaaltje geplaatst en onder een microscoop geanalyseerd. Het proces is een uitdaging vanwege de vervuiling door andere cellen en deeltjes en als de embryoloog het sperma mist, daalt het succespercentage.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">De AI-tool SpermSearch is ontworpen om dit proces te versnellen. Het team trainde het AI-algoritme met behulp van duizenden microscoopbeelden met gelabelde spermacellen. De AI-tool leerde hoe sperma eruitzag door beeldanalyse.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Het team gebruikte zowel gezonde spermamonsters als testiculaire weefselmonsters van zeven pati\u00ebnten met de diagnose NOA voor het testen.<\/span><\/p>\n<p><span style=\"font-weight: 400\"> Deze pati\u00ebnten, in de leeftijd van 36 tot 55 jaar, hadden eerder in de kliniek een chirurgische spermareportatie ondergaan en doneerden restweefsel dat was voorbereid voor spermareportatie maar niet was gebruikt.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Het AI-systeem werd vervolgens vergeleken met een embryoloog in een tijdproef voor sperma-identificatie en nauwkeurigheid. De AI identificeerde sperma in elk bekeken druppelgebied in minder dan een duizendste van de tijd die een embryoloog nodig had. Het systeem vond 60 meer spermacellen en was 5% nauwkeuriger dan de embryoloog per zichtbaar druppelgebied.<\/span><\/p>\n<p><span style=\"font-weight: 400\">De onderzoekers waarschuwden echter dat hun studie gebaseerd is op een proof-of-concept test en dat er een klinische studie nodig is om de doeltreffendheid en prestaties van de techniek bij het detecteren van sperma te garanderen.<\/span><\/p>\n<p>Carlos Calhaz-Jorge, professor en voorzitter van ESHRE, niet betrokken bij het onderzoek, zei: \"Voor mannen bij wie niet-obstructieve azo\u00f6spermie is vastgesteld, is ICSI met sperma uit de testikels de enige realistische kans om biologische kinderen te krijgen. Dit is een voorbereidend onderzoek naar het gebruik van AI voor het vinden van gezond sperma bij mannen met dit type onvruchtbaarheid. Het vinden van gezond sperma onder de microscoop in fragmenten van testiculaire biopten kan een moeizaam proces zijn. Het vooruitzicht om AI te gebruiken om het proces sneller en nauwkeuriger te maken is erg interessant.
